How do I request a copy of a birth, death or marriage certificate?

For all vital record copies you need to bring these documents to the City Clerk’s Office:

1. Picture Identification on the person requesting the document.
2. If the document is not for yourself, proof of lineage to the person/people named on the record.
3. $15 fee for the first copy and $6 for any additional copies at that time.
